To evaluate the sympathoadrenal system is a key point in the present concept of the pathogenesis of chronic heart failure. Today methods for evaluating the sympathoadrenal system in patients with chronic heart failure, which are used to measure plasma norepinephrine and epinephrine and to study cardiac rhythm variations, cannot assess the distribution and activity of the sympathoadrenal system just in the myocardium. Only radionuclide methods can provide a clinical imaging of the distribution of myocardial sympathetic innervation and its assessment now. For this purpose, 123I methaiodobenzylguanidine (MIBG) myocardial scintigraphy is mostly used. Global experience with this technique has been gained for more than 20 years. However, our country’s experience has been accumulated only for several years. The authors expose a broad section of Russian cardiologists to a brief account of experience with MIBG to diagnose, predict, and evaluate the efficiency of treatment in chronic heart failure.
